Bug description:  header documentation should be improved by moving the usage warning

Description:

From watching the php irc channel and mailing list the most common problem
people have when using the header() function is that they set the header
after sending content.

The documentation does address this issue however it's buried so that it's
far harder to find than it should be.  It currently sits between an example
for caching and an example for content type.

I would recomend moving the paragraph and example to the top of the
documentation.  Just above the first note: looks good.
