There is some useful information on building Tcl/Tk on MacOS X on the
Tcler's Wiki:

It is extremely straightforward in practice, because Daniel Steffen
maintains a comprehensive Makefile which handles pretty well every
eventuality. Obviously, you will need Apple's developer tools installed,
and MacOS X 10.2 or later, but those are the only prerequisites.
It would be interesting to know your experience with the patch.
